Obviously your company policy is missing some procedures that can prevent credit snatching. You could initiate these procedures yourself in your own Department to set an example to others including your boss. This is what I have found to work well.
1) Insist that all documents carry the initials of the person who created the idea (as the last line), even it is signed by his/her boss. This has the twin advantages of giving credit where due or to put the blame where due if things go wrong.
2) Always give your boss the best advice you can, based on your assessment of what is best for your company.
3) Keep a note of your contributions. Insist that these are included in your annual performance review. If your boss refuses to mention these in your assessment then bring these up in the panel review or to your boss's boss at the time of your periodic performance appraisal.
Note: If your company does not have a democratic performance appraisal system then credit snatching can never be controlled.
