BAs are very, very rarely asked to leave before the end of their two-year program - it would take, as you say, a major screw-up.
But there is no associate "program" or fixed length of stay. I've seen people designated as EMs as early as 18 months in and as late as 3 years. Performance evaluations are ongoing, with formal evaluations taking place for everyone (below partner level) semi-annually. So, every six months, each consultant is put into one of 5 categories, the bottom one of which is "counsel to leave." Basically, after you've been at a particular level (associate or EM, anyway - not sure how it works for APs, but probably similar) for a year or so, they can counsel you out if they feel your performance is not up to par and you're not on a trajectory that will get you where they feel you need to be in a reasonable amount of time.
