1. It's irrelevant or should be. You will get some people here who think otherwise though. But as long as you can get yourself into an interview, what matters is acing the cases. If some dick asks you why you didn't do an internship, come up with some answer like "just heard about it" or "my major doesn't often go into consulting" or "just learned of consulting appeal". (i.e. just have some crisp prepared line.) Keep it short and sweet...move on.
2. Donno. Imagine it would not matter much.
3. I've heard it is good. Wetfeet press is ok too. Another source is MBA-school consulting club case-books. Another source is firm case workshops (often at B-schools).

By the way, one further question: how much time is spent travelling, and how gruelling is it? Sounds like the best part of the job, but somehow I think things might be different once your doing it...
