Some boutique, or more niche firms in Financial Services:
First Manhattan
Oliver Wyman
OC&C (recently launched FS practice)
Others with different focus:
Kurt Salomon (retail)
Roland Berger (v. german)
PA Consutling (pred. UK)
There are others, but I know mostly of European ones- FMC, Oliver Wyman, and OC&C are probably the most well respected internationally out of the boutique strategy firms.

I would add
ZS associates (pharma, sales)
Marakon (value-based management)
Stern Stewart (economic value added)
Partnering Group (grocery)
There are also dozens of small industry-focused consultancies, like Kurt Salomon (KSA) in retail. While "vertical" firms like KSA have great industry expertise, the problem for junior consultants is that the work tends to be exactly the same--a single idea sold to as many companies as possible before the idea reaches its "sell by" date. Moreover, the big enterprise-level strat engagements will still go to a big strat firm. However, for individuals who really enjoy one particlar industry or one functional area, the boutique is the best bet.
