I'm not Sam, but I am a 2L at a top ten school.
Associate positions are available for 1Ls, but nearly all the 2Ls I know who worked for law firms last summer had (1) science or tech PhDs or (2) connections to the firms they worked at (e.g., family). I don't know of anyone who worked for a firm last summer who did not fit into one of those two categories, but of course that doesn't mean such people don't exist.
I did an externship with a federal district judge last summer. I sent out my applications in mid-December and had a spot by end of the year. I sent around 40 applications to judges in the area where I wanted to work. Include a cover letter (emphasize any research and writing skills in your background, because that's basically all you do as an extern), resume, and writing sample. If you have grades by the time you apply, send your transcript too.
You don't need to know what you want to get into.

to add to Nozzle's comments, those that got top 1L jobs were also often: 1) minorities from schools that the firm is targetting 2Ls at 2) people with just ridiculously good grades 3) sometimes former D.1 football/bball players. If you're not at a top 10 it will be pretty darned near impossible to get a $2400 a week job as a 1L. (I'm basing this on my 1L firm's hiring patterns and those of my peers). Make sure you send out your letters on December 1st, but don't count on anything because the larger markets know you'll apply next year anyway. You will probably have better luck at secondary markets that you have connections to (St. Louis, Seattle, Pheonix, etc). Good luck
