Flaked out of a Community College Class |

I took an intro Accounting class in undergrad at a community college around my undergrad institution.
No kidding, I don't remember the name of the school, I don't even remember if I got a grade. It was an independent study class and I totally flaked, I think I took one exam and then didn't do any of the rest of the work and I think it wound up going down as an incomplete. But I can't be sure. I never turned in anything to my undergrad (not sure there was anything to turn in) and like I said, I can't even remember the name of the college. Obviously, its not on my LSAC report either.
Anyways, I'm working on my GW app right now and they want to know of any and all institutions attended whether the work was for credit or not. Most other applications only want to know about full time study and don't even care about any summer school.
Anyone have any advice on what to do? I completely forgot about this class until I read the GW application. Not quite sure how to go about this. Thanks.
