A couple of questions
a) When it comes time for my questions for the interviewer, is it ok to take out a prepared list? I read somewhere that it shows preparedness, whats everyone's take on this?
b) when getting answers to my questions, is it a good idea to take brief notes on it, or should we just remember everything?
c) if the interviewer offers you coffee or some other food/drink, should you accept out of politeness or always decline?

Generally no.
But if you're at one of those three hour interviews where you talk to six different people, and you need a drink after two hours of talking... well I think you're entitled to that and no one is going to hold it against you.
