A couple of questions
a) When it comes time for my questions for the interviewer, is it ok to take out a prepared list? I read somewhere that it shows preparedness, whats everyone's take on this?
b) when getting answers to my questions, is it a good idea to take brief notes on it, or should we just remember everything?
c) if the interviewer offers you coffee or some other food/drink, should you accept out of politeness or always decline?

Yes, always ask permission to take notes, preferably at the beginning of the interview, so you'll be ready & you won't disrupt the flow of the interview by asking later.
No, never accept food or drink, lest it distract you from your main task, or worse yet, result in a spill/talking with mouth full, etc.
Mebbe a list would impress some, but could be a negative with others. If they want you, the list won't be a big issue, & if they don't, your goose is already cooked.
