out of curiosity, are analysts ever fired? Either during training or afterwards, for simply not being good at what they do (or perhaps sleeping with their boss' wife?).

Yes - didn't you hear about JP Morgan in 1998? This was when the Russian financial meltdown occurred. JP Morgan has 3 analyst training start dates, and they fired a bunch of analysts who were still in training in the October start date!
They just moved to NY and then were fired in training. I think they based it on performance in training, so it wasn't 100% random.
I think a JP Morgan analyst was fired for fudging expense receipts.
With the exception of the 1998 JP Morgan sitution, you have to do something glaringly wrong to be fired as an analyst.
