I run a Central Billing Office and you're absolutely right. The balanced budget act of 1997 slammed the medical profession with reduced fee schedules (which other carriers quickly followed). Additionally stepped up legal action on compliance has added another complication.
Essentially Doc's give away a high amount of service and this was being subsidized by HCFA and insurance carriers. When they stopped the subsidy, the patient still needed care (irrespective of ability to pay).
The pressure point was the physician and continues in this profession.
Managed care is another treadmill for physicians. Expectations and reality are a real problem in this industry.
