I signed up for this health plan and it says on the benefits site that I get 100% coverage for eye exams and $150/2yr for lenses and frames in or out of network. I got my exams and hardware all done out of network and I can't find any information on United Health's website on how to get my money back. Does anyone know how to get reimbursed for these costs? |

First, you pay for stuff then keep the receipt. Send the receipt and a form on the UHC ppo website. You will get back your money after a couple of months.
Second, DON'T use the vault for such a stupid question. United Health Care has a 1-800 NUMBER like every other company in the universe to ask questions....
